Support Broker Services

Self-directing means you choose your workers, their schedule, and tasks, while managing the payment. There are rules to follow to protect you and your workers. Support Brokers help you stay informed about these rules.

Choice and Control, Inc. can guide you in making decisions about your plan, services, and budget, and offer advice on being a great employer.

We Will

• Help you make decisions about your goals, needs, services and resources.
• Help you get and manage the services you choose.
• Follow your Person Centered Plan and follow laws about your Medicaid Services.
• Keep records of employment and financial information, like timesheets and invoices.

We Cannot

• Provide other paid services in addition to Support Broker Services to you.
• Do the work your Coordinator of Community Services (CCS) is required to do.
• Make decisions for you, sign your name on service delivery, approve our bills, or hire or fire the people who work for you.

We will inform, coach, and mentor you on other aspects of your Self-Directed Services.
